+(ISSUED: 07/01/2009 @10:30 AM - Problem with the processing of CPT critical care codes 99291 and 99292 when billed with a minor surgical procedure (click for details)

We have identified a problem with the processing of CPT code 99291 (Critical care; first 30-74 minutes) and 99292 (Critical care; each additional 30 minutes) when billed on the same day of a minor surgical procedure. The critical care when billed with a 25 modifier may have inadvertently denied. The problem has been resolved and any impacted claims will be automatically adjusted.

Highmark Medicare Services is the Part B Medicare Administrative Contractor for Pennsylvania (PA), Delaware (DE), Maryland (MD), New Jersey (NJ), and the District of Columbia Metropolitan Area (DCMA). For Part B services, A/B MAC Jurisdiction 12 includes the Counties of Arlington and Fairfax in Virginia and the City of Alexandria in Virginia.
