If you are looking for active LCDs and are in one of the regions listed below, you should only use our policy search BEFORE the LCD effective date listed below.
After 12/12/08, all legacy policy documents will be archived, and our policy search application will be activated for the new J12 MAC policies for all regions.
During our MAC J12 Transition, Highmark Medicare Services will house both legacy/current Local Coverage Determinations (for our Pennsylvania Part B and Pennsylvania/Maryland/Washington D.C. Part A lines of business), along with our new MAC J12 Local Coverage Determinations.
Which Local Coverage Determinations should I use?
Use the following schedule to determine when you should begin using our J12 MAC Local Coverage Determinations. The effective dates for these policies will depend on your provider type and region:

Now, Make Your Selection...
- I have now transitioned to the J12 MAC and represent Medicare Part A for New Jersey Pennsylvania, Maryland, and DC, OR Medicare Part B for Maryland, Washington DC, or Delaware.
- I have not yet transitioned into the J12 MAC, and I need to access Highmark Medicare Services Local Coverage Determinations still in effect prior to the MAC transition. Take me to the current policy area for Medicare Part B.
